Case Study: Jason’s Journey to Independence
Jason (not his real name) began working with Transform Community Development following a challenging period marked by mental health difficulties and a relationship breakdown. At the time, he was residing at our temporary accommodation unit at 10 Brewery Lane.
Identifying Needs
Together with housing support staff at Brewery Lane, Jason identified key areas where support was needed:

Accessing a safe and secure tenancy

Accessing mental health support

Addressing social isolation
Initial Support and Progress
Jason was supported to apply for local authority housing and underwent a benefits check to maximise his income. He was also linked with the Community Mental Health Team (CMHT) and began receiving regular support from a Community Psychiatric Nurse (CPN) twice a week.
Recognising that Jason was not yet ready for a mainstream tenancy, he was then moved to our Seagate Project, which provided 24-hour supported accommodation. Here, staff worked closely with Jason to develop essential life skills such as self-medicating, budgeting, and engaging with the wider community.
Moving Towards Independence
In December 2023, Jason transitioned into his own mainstream tenancy within sheltered accommodation. Staff provided intensive support five days a week, helping Jason build his confidence, life skills, and independence. His mental wellbeing improved significantly, and he was discharged from the CMHT due to this positive progress.
Support was gradually reduced to twice weekly as staff observed Jason coping well in his home environment and benefiting from a strong family support network.
Outcome
Jason now confidently maintains his tenancy independently and has successfully graduated from the service. He continues to thrive as an active member of the community, demonstrating significant growth in his confidence, independence, and wellbeing.
